The MIND-USA study explored the long-term effects of antipsychotic medications—haloperidol and ziprasidone—on critically ill patients experiencing delirium. This follow-up analysis was conducted at 16 U.S. hospitals, focusing on adult ICU patients with respiratory failure or shock who developed delirium. Participants were randomly assigned to receive haloperidol, ziprasidone or a placebo intravenously for up to 14 days. Outcomes were assessed at three and 12 months post-randomization, examining cognitive, functional, psychological, quality-of-life and employment metrics using standardized telephone assessments.
Of the nearly 21,000 individuals screened, more than 560 met eligibility criteria and were enrolled. At follow-up, all three groups showed similar survival and response rates. Cognitive impairment remained common across all groups, with approximately one-third of patients affected at both three and 12 months. Over half the survivors in each group were unable to return to work due to cognitive or physical limitations. Importantly, statistical analysis revealed no significant differences between the placebo and antipsychotic-treated groups in any measured long-term outcomes. Adjusted odds ratios showed minimal effect of either haloperidol or ziprasidone on cognitive scores or other quality-of-life indicators.
The study concludes that haloperidol and ziprasidone do not improve long-term cognitive, functional or psychological outcomes in delirious ICU patients. Given the lack of both short- and long-term benefit and concerns over continued antipsychotic use after discharge, the findings support avoiding routine antipsychotic administration for delirium in critically ill adults. These results have important implications for clinical guidelines and prescribing practices in intensive care settings.
