Dr. Jennifer Carnahan’s research focuses on improving care for older adults in nursing homes, most recently focusing on transitions of care from the skilled nursing facility (SNF) setting to home for patients with Alzheimer’s disease and related dementias. This research interest has evolved from her medical training as a geriatrician.
Dr. Carnahan completed a two-year research fellowship at the Center for Aging Research and then joined the faculty in 2016.

The Coming Home After Rehabilitation and Transition from a Skilled nursing facility with Dementia (CHARTS-D) Study

(K23 – NIA) (2020-2025)

Role: Principal Investigator

The CHARTS-D study is a career development award that uses a national secondary dataset to examine the relationship between cognitive impairment and outcomes of patients who undergo the complex set of transitions from hospital to skilled nursing facility and back to home.
